For all other medical record requests including: inpatients at St. Vincent’s Midtown Hospital or St. Clare’s hospital, Family Health Center Clinic (at 51st street or at our Washington Heights clinics), Dental clinic, ambulatory surgery, Radiology, Cardiovascular Center, Dialysis, Physical Therapy or of any other outpatient service please call MEDREX, our post-dissolution records custodian at 845.338.2200
